Digital health technologies (DHT) such as wearables, mobile devices and home-based sensing systems are poised to deliver precise, convenient, remote monitoring solutions to enhance and modernize clinical research, clinical care and promote opportunity for self-management. The field of movement disorders and especially Parkinson's disease (PD) have been early adopters of DHT driven by the pressing need for better solutions to manage chronic, complex, fluctuating and heterogenous conditions. In this chapter we explore the challenges and opportunities for remote monitoring as follows: identify the opportunity for remote monitoring with the broad aims to set out the current approaches for remote monitoring in clinical research; explore evidence for implementation of remote monitoring in clinical care and self-management; summarize challenges and solutions to implementation; and, set out the next steps and a vision for future digitally enabled research and care pathways.
